The only failure iIve seen has been with a standard AA S200T which blew the face of the gauge out across the the back of a range during what could be described as an over enthusiastic fill. Because it was being filled with the gauge pointing away from anyone and it being a light object it didn't injure anyone.
I'm not sure we're dealing with a volume of accidents that befits the huge amount of effort and cost to ban 3rd party modded and made parts.
